There are certain sentences using 'kvar' which can't translate well to English using 'already', nor can I think of any other word immediately equivalent to kvar. They seem to use kvar as an intensifier. Can you think of any good ways of translating such sentences into English, for example sentences like 'החיים זה כבר לא הכול', 'זה כבר לא מצחיק'?

כבר is either "already" or "anymore". I both examples you've given "anymore" would fit.

How about 'זה כבר שנים שלא אמרתי לך', 'מחכה כבר שתהיה לי יומולדת'?

You're going deep into the language now and you're right, it cannot always be 1:1 translation.
As for birthday, I would use can't wait anymore/longer,
the second sentence needs a whole lot of surgeries to be translated, rephrased.
