Is the 'female slave' שפחה always used whenever the slave is female, or does "slave" have no (modern) distinct feminine form to be used?
Can שפחה be used in a metaphorical sense, e.g. she is a slave to addiction/desire/fate?

שפחה is the word for female slave. Biblical Hebrew also has אמה. There is no derivate from עבד if that's what you mean.

    I don't see why not.
