    This is from a medical questionnaire, which can be addressed to both the patient directly or a family memember or friend of the patient(an informant):

    Al paciente, ¿le han resultado difíciles los cambios en estas actividades? ¿Ha mejorado, ha empeorado o se ha mantenido igual?


    I'm not sure if the above means:
    To the patient, have you found the changes in these activities dificult? Have you improved, got worse or stayed the same?
    or
    Has the patient found the changes in these activities difficult? Has s/he improved, got worse or stayed the same?
    i.e. the question is addressed to the informant

    Any help or ideas will be greatly appreciated.

    The second one is more accurate (in my opinion) .

    If there is a comma after "al paciente", I would go for your first interpretation.
