I know bordereau usually means "slip" or "form."
I'm in charge of translating the following french text into english and it's full of unfamiliar terms. Merci d'avance : )

When an employer established in France or a foreign country has already applied for a work authorization, the service instructor can as him to produce the last receipt of payments and social contributions addressed to the institution charged with their recoverment (by default of independent naming certificate) and, if necessary, to the paid holiday fund, relative to the employment of this (or these) worker(s).

No. I would use "form" in this case.
Moreover, a "receipt" is something one.... receives (most of the time). Whereas a "bordereau" is something one issues.
