In which cases to use "fena" and in which "kötü"?
Can't think of a context where they wouldn't be interchangeable.
In most cases they are interchangeable to mean "evil bad, nasty" although; in some cases "fena" is lighter than "kötü" as in "o fena bir çocuk." which means "he is a naughty/peevish boy".
Separate names with a comma.
