This may seem a stupid question, but as a student of Italian, the usted form often eludes me. How would one say, "May I help you" My instict is to use "¿Puedo ayudar(lo/la) ?" or would one use the reflexive 'se'?

There is no need to use the reflexive pronoun here. It is right as it is.

May I help you? = ¿puedo ayudarte (a ti), puedo ayudarle (a usted), puedo ayudaros (a vosotros), puedo ayudarles (a ustedes)?

Sí, en ambiente informal. Pero DareRyan ha preguntado por el uso formal (de usted).

    ¿Puedo ayudarte (informal) /le/lo/la (formal)?

Most people use le/les rather than the correct form lo(s)/la(s)
