I wonder if there is a more formal way to say it. Is there any?

Perhaps "I am never sure when to use house or home".

How about: "I always confuse the words x and y." ?

I always confuse the words house and home.
I always have difficulty distinguishing between the words house and home.
Invariably I find it difficult to distinguish the word house from the word home.
Distinguishing the word house from the word home poses a perpetual dilemma for me. :)

Lol, perhaps something simpler will suffice. Lo que han sugerido SydLexia y OldyNuts están bien.
