    se non avete reinviato il modulo in tempo utile....

    KG is right, but the rest of the sentence is difficult without context.

    If you haven't returned back the accomodation form in time , we could suggest you another hotel

    If this is something that may happen in the future, you can say:

    If we do not receive your accomodation form in time, we may be able to suggest another hotel to you.
