Do you translate this as "De qué tienes ganas de comer ahora?" or "Qué tienes ganas de comer ahora?" or some other way? Nothing I come up with sounds right to me.

¿Qué tienes ganas de comer ahora? is correct.

But what I'd usually say: ¿Qué te apetece/te gustaría comer (ahora)?

I would probably say, "¿Qué te gustaría comer?" or, "¿Que te apetece?"

In Mexico we would say ¿Qué se te antoja comer?
