I have a company establishment legal document but I am unsure about some fields.
I am having difficulty typing these in chinese so I have highlighted in red the fields I am unsure about.

There should be standard translations of the names of IDs. Here I am just giving their meanings, which may not be their standard English names. I'm also not 100% sure about what 葡籍認別證 means.

Line 1:
持有(Holding) 居民證(Resident ID) 葡籍認別證(Portuguese Certificate) 其他(Other)
You should check one of 居民證, 葡籍認別證 and 其他, depending on which ID you have. If you check 其他, you should give the name of the ID in the blank after 其他.

Line 2:
編號(Number) 於[date]由...簽發(issued by ... at [date])
Fill in the number of your ID. The issue date should be in the 日月年 blanks, and the name of the organization issued your ID should be put into the last blank.

I would say 葡籍認別證 could also be translated into Portuguese National ID Card.
