Hello teachers,
This the website I took the sentence from: English Language Center

How will you explain what a sponsorship form is. Will the following definition work?
A document in which a sponsor signs and agrees to give money to someone, in this case to the student.

Sentence:
You would need to send in a $35 non-refundable application fee, a sponsorship form indicating who will be responsible financially for the student while studying in our program.

    Although that is a definition which usually works for "sponsorship form", in this case it is a little more precise - it is a form in which the person who signs says that they will provide all financial support necessary for the student - the form will generally list whether this includes accommodation, food, tuition fees, etc.
