I would say:

Se avete perso i dati di accesso al vostro account, potete recuperarli nella pagina Sign In (or di ingresso al sito?) di *** seguendo questo link (= if the link redirects to the sign in page)

Hi, first of all I would like to say that many italian sites contain english words such as password, account, userid and username (nome utente), home page etc., that is they are not translated at all. So I think you should not translate everything, it would just sound funny. Also, I guess you mean you can't open a new account giving the same email address. Please be specific.
A possible translation would be:
Se siete utenti già registrati, non potete aprire un altro account con lo stesso indirizzo di posta elettronica. Se avete dimenticato le informazioni del vostro account (userid, password...), potete ottenerle andando alla pagina di registrazione (sign in) del sito ...... Cliccate per favore i seguenti links per accedere direttamente alle pagine desiderate.
