Are any of the above incorrect?

Which of them is more appropriate?

    They are both OK.

    I do not know if there is any context which might make a difference. Where are you planning to use this?

    In a short story.

    All he had to do was to wait. vs All he had to do was wait.

    In this case, isn't the first sentence incorrect?

    They're both commonly used. You get the same choice with several other expressions:

    What he wanted to do was (to) wait.
    The best thing to do is (to) wait.
