The setting is a one-on-one conversation between two people. One says "lo siento." I'm sorry. What does the other person say in response? Thank you...

    Disculpa aceptada or acepto su disculpa.

    It's far common in plural (disculpas aceptadas) . I think you will never hear that in singular.

    Hay varias opciones, dependiendo de la gravedad del asunto:

    - Lo siento.
    - Está bien.

    - Lo siento.
    - No hay problema / No te preocupes.

    - Lo siento mucho(and crying :p).
    - Te perdono.

    Etc.

    The first person is apologizing (lo siento) to the second person for having accused him of an error he was not responsible for making :).
