I can't decide whether to leave it in English or is there anyway that it could make sense in Spanish?

Applicant Data Record....this is regarding the "survey" where applicants put whether male/remale, or what race or ethnic group they fall into.

The way I see it: In a survey the applicant is called "encuestado" in spanish and applicant data record could be also named: "applicant demografics".

    In spanish you could say:

Datos del encuestado or Datos demográficos del encuestado

    Hope this helps.
