    I think the phrase includes the idea of a comparison or level.

    ...and who half the time do not qualify for any specific psychiatric diagnosis, as disturbed as they are, according to an analysis...

    ...and who as often as not do not meet the criteria of any specific psychiatric diagnosis, even though their level of mental disturbance is obviously high, according to an analysis...

    as often as not = the frequency that it happens reaches the level of 50% of the time, it happens as often as it doesn't happen
