How would I say:

"Dear <>
I would like to buy an autographed photograph of you. It would be great if you could put the following words on the photograph:

Please let me know if you could do this and how much it would be with postage to the USA.
"

    My attempt:

    Caro__________,

    Vorrei comprare una foto di te con autografo. Sarebbe bello se tu potessi scrivere queste parole sulla foto.

    Per favore, fammi sapere se saresti disposto a fare questo e quanto sarebbbere le spese postali per gli Stati Uniti.

    Wait for other suggestions, especially from native speakers.

    Per favore fammi/mi faccia sapere se puoi/può farlo e quanto verrebbe a costare spedirlo negli Stati Uniti."

    I'm not sure I understood the last sentence correctly: are you asking how much it costs to send it to the USA or how much does it take to send it?
    PS I put both formal and informa adress.

    Actually, what the total cost would be for the photograph and postage to the USA.

    Also I'm asking one person for a photo of themselves, so I'm not sure which of the alternatives on either side of the "/" to use below:

    Since you don't know the person you are writing to, I would opt for formal:

    Per favore mi faccia sapere se può farlo.

    It's not a matter of one person or many; it's a matter of formal v. informal.
