Hi, I am writing to make sure what's the best way to describe this. It is about a birth certificate. I was wondering if Birth certificate of Jhon Smith is better description than Birth certificate pertaining to, what do you think? It's a formal document and I have to describe in the first page the document content.

    Is the formal document a birth certificate?

    If yes, I would simply say: This is John Smith's birth certificate.

    Otherwise, please add more information.

    Thanks for answering, actually It would be added as a title of a document (a French into English translation of a birth certificate). It would appear in the first page thereof.

    In that case, if it's the title and you don't have to write a complete sentence, you could write:

    John Smith's Birth Certificate

    We do capitalize titles.

    "John Smith's Birth Certificate" is not a title that would appear on the actual document. In describing the document, which is what the OP asked about, "birth" is not capitalized.
