    Since 'it' doesn't get translated here, that is more likely to be interpreted as "she will break my heart", but in this case I guess it would not be inappropriate. Alternatively, you could say "mi si spezzerà il cuore". (my heart will break)

    You can very well say Se mi dice: "No", but with and No there is also an extra idiomatic way to say it: "se mi dice di no" (or di sì).

    Spaccare is not used for the heart. Moreover, take into account franx's comments about "dire di sì/no".

    Se mi dice di no, mi (si) spezzerà/spezza il cuore.

    By adding a "si" you translate more closely the fact that you said "it will break my heart" rather than "she will break my heart".
