Is there a particular reason you said "one more"? Had one appointment, or some appointments already been booked and you wanted to book an additional one? If not, you could just say, "I'd like to schedule an appointment with the doctor for [my son/my neighbor/my mother-in-law]"

Saying "one more" is wrong. That means "add a second appointment".

If you are on the phone, you would say "I want to make an appointment with Doctor Jones for Judy Smith".

Then the appointment will be Judy Smith seeing Doctor Jones.
