This is the title of a quality control document listing production defects on products related to the packaging industry. It is formatted as a letter, so this may just be a descriptive phrase that merits a literal translation, but I'm wondering if anyone has ever come across it?

Based on your description, I can't imagine that it is a letter (To whom it may concern...). I would say it's more likely to be a report. If it contains any tables or other such graphics, it might be a chart.

I'd go with "defect report."

Hi gengo - While there are report-like elements, it actually is framed exactly like a letter ("A quien interese"), and originates with one of the company's clients. Something between a report, as you say, and a letter of complaint for non-compliance.
