Suppose that Tom was in love with Mary. Since he met Jill, he has been in love with her. So, he decides to break up with Mary.

(ex) Tom has changed his heart from Mary to Jill.

Does it make sense to say: change one's heart from someone to someone else? If not, what is the proper way of saying it? Thanks a lot.

"Changed his heart from Mary to Jill" doesn't sound very natural to me, BF. If I wanted to use "change" and "heart" to express the idea, I'd use this version: Tom has had a change of heart. Now he loves Mary rather than Jill.
