I am confused with this word.

Your name appeared by default in my system.
Your name appeared as default in my system.

I don't know if I should use as or by default in most case.

I would typically use "by default" and "as the default", but it's an odd sentence. A name was automatically filled in somewhere in your system? "By default" in a software environment would mean that some particular name appears unless another action is taken.
