    Does the bold sentence imply:

    1. Each chapter includes [an introduction of major works by specific artists] and [a survey of major works by specific artists].

    or

    2. Each chapter includes [an introduction] and [a survey of major works by specific artists].

    or

    both of them are possible?

    In general, chronology is the organizing principle for this study. Each chapter includes an introduction and a survey of major works by specific artists. As much as possible, an endnote cites an accessible location – i.e. institutional, not private – for each work. All the artists in the book appear in the index, where the entry provides their birth and death dates. The index also refers to all cited works (Art and War by Laura Brandon).

    It's the second meaning. (but if you are not convinced, you can always leaf through the book and see for yourself how each chapter is organized)
