economic outpatient care

"Their parents did not provide economic outpatient care." (William D. Danko, The Millionaire Next Door)
I don't quite understand what the author means by "economic outpatient care" in the sentence above. I believe the word "outpatient" is suppose to be understood figuratively in this situation but still don't know how.

    You'll notice that the author then says he'll explain each of those points. There's a whole chapter explaining what he means by "Economic Outpatient Care" starting at page 140 on the edition on Amazon.com.
