Hi,
Here are some words from the novel Lady Chatterley's Lover(para. 31) by Lawrence(the University of Adelaide,here):
He looked at her with his keen blue eyes. He seemed kindly, but distant. But at least he was sane, and wholesome, if even he looked thin and ill. A cough troubled him.

I feel "wholesome" and "thin and ill" are contradictory. What's your opinions please?

    wholesome -> physically healthy

    But at least he was sane, and wholesome, <- this was the reality -> He really was sane and physically healthy.
    if even he looked thin and ill. <- this is how he appeared. His appearance was deceptive in this respect.
