How are you? Bad or not very good?

Hello, everyone:

My question is which do you use more often in American English?, understading they may mean the same.

Actually, I have not heard "bad" as a reponse, but it's also possible, right?

    I agree that "bad" is a valid response but it's a strong one and probably not as common as others. In the Midwest, a common response is, "I could be better," which means the same as "not very good."
