Sorry for my late reply. I had not checked my email for ages.

Should the verb be in the past perfect form?

The sentence as you write it is grammatically correct. Is there a context or accompanying text that would lead you to question that?

(NB: As an AE speaker, I would find "I had not checked my email in ages" slightly more idiomatic.)

The alternative, "I have not checked my email in ages," would mean that you still have not checked your email. If you are replying to an email, it is obvious that you have done so.
