Hello,
hope someone can help me please. Is this sentence correct?

The opportunity presented is very interesting and I would be grateful if it is possible to send me an application form.

Shoud I use if it was possible?Would it be better?

Thanks a lot :)

You should use "I would be grateful if it were possible to send me an application form" if you want to use that construction. "If" takes the subjunctive. Alternatively I would say "I would be grateful if you could send me an application form" as this sounds more natural.
