I have a question regarding the phrase "in nota" in the following (from a patient's cartella clinica):

"Paziente in nota x intervento Ia della lista"

Could a possible translation be the following:

"Patient is first on the list scheduled for surgery." or "Patient is scheduled for surgery, first on the list."
