Although I have looked 'incumbent' up in a dictionary (1. currently holding an indicated office 2. obligatory 3. resting, lying, or pressing on something) I still can't understand its use altogether with 'symptoms'. Perhaps someone can help.

    Since I am, it seems, not allowed to give the author's publicly available email, I suggest that you look him up on the internet, where you will easily find it. If he writes phrases like the one you quoted, I would say that it is incumbent on him to provide some explanation.
