Letter: Heading

In a beginning of a letter, above on right I have to insert:

name, surname, address
date

or

address
date
(and in this case insert my name and surname only at the end of the letter)

    I'm not sure exactly your question(s), but I can give you a common, example letter format for you to copy.

    Thank you, it's ok.

    The information I've found were wrong: a web site in fact suggests to insert in the heading of the letter the address of sender, but without name and surname of sender.

    A great deal depends on the kind of letter you are writing.
    A personal letter from me to you would look like:
    ........................My address
    ........................Today's date
    Dear niela,
    Letter, letter. letter .....
    Letter letter letter.
