In the story I'm translating about a Captain in the 17th century, I could give a right sense to this sentence. I need your help.

"Looking every inch the gentlemen, John lifted his right hand and offered it to Ann in greeting".
"Come i veri gentiluomini????, John alzò la sua mano destra e la offrì a Anna per salutarla".
