A name tag as in just any time someone has a tag on their shirt with their name - it may be plastic pinned on, or a sticker, or sewed on - like at a restaurant, or store, or a party.

You don't need any article, just the name in the nominative case--for example "Γιώργος Παπαδόπουλος" or "Μαρία Παπαδημητρίου".
