As an example of the below sentence:

The contract provides that the landlord pay for upkeep.

1. Does the verb that comes after (that) comes in simple present form?

2. Why the verb is pay not pays?

    It's the subjunctive. In the third person subjunctive the verb does not take an "s." So in effect it's the same as the infinitive without the "to". "provide that" is one of the verval expressions that calls for the subjunctive, though not everyone may use it.
