Could you please help me, I´m trying to translate the following sentence (it is part of a policy application), but I´m not sure if this is a complete sentence, or if it is divided, I have no idea how to translate this to Spanish.

Insurer Policy Type Deductible/Retention Period From/To Premium

My try is this:

Tipo de Deducible/retención de poliza de la aseguradora…....Período de/a…... Prima

But I divided the English sentence:
Insurer Policy Type Deductible/Retention...... Period From/To....... Premium

This is the complete idea (as it is written in the document):

I´m not sure my translation is correct.
