Hello all!

I'm translating a formal certification from spanish into english and I would like to know if the following sentence is correct or if there's any other better way of saying the same (bold words):

The undersigned, Mr. JOSE ARNER BUENO, notary of this Archbishopric of Zaragoza, appointed by Archbishop Mr Elías Yanez Álvarez, in order to publicly bear witness to the personal and free statements of relinquishment of the Christian faith in the diocese of Zaragoza, STATES THAT
