SMI [serious mental illness]

I have not found a Spanish equivalent for the English acronym SMI, used in mental/behavioral health field for "serious mental illness"

Source (on an intake questionnaire):
Have you ever been designated as having a SMI?

Here is my attempt:
¿En alguna vez se le ha diagnosticado con una enfermedad mental grave?

    It makes sense and I have seen it used, but I usually go with "trastorno mental." Remember, it's up to the patient to ask questions if they don't understand it and it's up to the provider to explain it.
