Steak, from well-done to rare

Hi all,
I have found a couple of forums that translate these words, but I was hoping that someone could please, once and for all, list all these in Spanish. They are referring to how you would order a steak.

Rare:
Medium-rare:
Medium:
Medium-well:
Well-done:

Also, if you could please tell me what country your translation is from, it would be much appreciated. (I don't know if these vary or not. I'm specifically interested in Spain and Mexico.)

    The following terms are for Mexico City.

    Rare: ROJO
    Medium-rare: MEDIO ROJO
    Medium: TÉRMINO MEDIO
    Medium-well: ???
    Well-done: BIEN HECHO o BIEN COCIDO

    I've heard a few other terms that I can't think of right now.

    I'm from Mexico. These are the terms I know:

    Casi cruda = Rare
    Medio cruda = Medium-rare
    Término medio = Medium
    Tres cuartos =
    Medium-well
    Bien cocida = Well-done

    En la Argentina:

    Rare = muy jugosa
    Medium rare = jugosa
    Medium = a punto
    Medium well = cocida
    Well done = bien cocida
