The primary meaning is alcohol abuse ("I am on the (water) wagon" means that I am trying to abstain from using alcohol or, by extension, from using other drugs). "Faire une rechute" (in English, "to relapse") seems fine for alcohol or any other drug. But it would be surprising if there were not a more familiar version (which I don't know).
