La idea de "to be top of the heap" es la de estar por encima de la gente promedio. En Cuba he escuchado "estar por encima del montón", en tanto el DRAE define en una de sus acepciones que:

ser alguien del montón
1. loc. verb. coloq. Ser adocenado y vulgar, en su persona o condición social.
