American English
a sight for sore eyes = A welcome visitor - someone you are glad to see, something you are happy to see, it implies something soothing... or it can be used sarcastically to mean the opposite.

E.g. What a sight for sore eyes!

How could you translate it in a positive and sarcastic way?

    The OP's suggestion:
she is a sight for sore eyes, ci si rifà gli occhi a guardarla'.
    the Hazon's suggestion:
sight for sore eyes, (fam.) un piacere a vedersi.
    A third option:
una gioia per gli occhi. :);)
