Hi, please see the following survey question I made up.

In the previous question, you answered that you bought a cold medicine in the last three months. What format was it?

1. Tablets
2. Capsules
3. Fine granules in packets
4. Other

Is "format" an appropriate term here?

    Form sounds better than format.
    It's the term used in the field and the wiki article includes:
    Depending on the method/route of administration, dosage forms come in several types. These include many kinds of liquid, solid, and semisolid dosage forms. Common dosage forms include pill, tablet, or capsule, drink or syrup, and natural or herbal form such as plant or food of sorts, among many others.
