When I can use "It" to answer "This, that, these, those" questions?

When I point out someone who I don't know, can I use "It" to answer the following questions:
- Is that a doctor? -> Yes, it is. / Yes, it is a doctor.
- That is a doctor, isn't it? -> No, it isn't. / No, it is not a doctor.

- Is that Tom? -> Yes, it is. / Yes, it is Tom.
- That is Tom, isn't it? -> No, it isn't. / No, it is not Tom.
- Who is that? -> It's Tom.

- Is that your friend? -> Yes, it is. / Yes, it is my friend.
- That is your friend, isn't it? -> No, it isn't. / No, it is not my friend.
- Who is that? -> It's my friend.

    The others are all right, but these three make no sense:

    - That is a doctor, isn't it? -> No, it is a doctor. :cross:
    - That is Tom, isn't it? -> No, it is Tom. :cross:
    - That is your friend, isn't it? -> No, it is my friend. :cross:
