Which one is right: 'I,Jessica,am not well' or 'I,Jessica,is not well'?

Tell me, would you say "I am not well?" or "I is not well"?
There are two possible ways to read "I, Jessica, am not well", but whether you are Jessica, or whether you are talking to Jessica, it is you who are not well.
Separate names with a comma.
